Caregiver
Caregiver can be a relative, friend or person in the community who is willing to live with the client and provide the needed care. Caregiver agrees to provide assistance, supervision and hands on care as needed, according to the client’s individual care plan.Nursing Services
The Registered Nurse and Care Manager assess all new Caregivers and members upon initial intake, 6 months & annually and upon any significant change in the member’s condition. The Registered Nurse as part of the multidisciplinary team provides on-going consultation.Care Management
The Care Manager is responsible for care management of all non-medical services to the member in the AFC program. S/he visits the homes monthly or bi-monthly to meet with the Caregiver and individual members to continually assess the program’s effectiveness.

We will accomplish this mission through partnership with the people we serve, with a highly skilled team of diverse and culturally competent staff. The AFC program is designed to offer a living option for individuals with a medical or psychiatric diagnosis who require assistance with their activities of daily living (ADLs) in or der to maintain a safe lifestyle within the community.
Health & Medical Services
On the other hand, we receive appropriate care and services and prompt medical care as needed.
